First, unlocking works only for new careers on the same profile and map. You can have 3 profiles, each with 5 career slots. For instance, if you have a career on Western Front map with unlocked 3rd rank (Feldwebel etc) and career on Channel map with unlocked 2nd rank (Vizefeldwebel) , every new western Front careers you make can start from rank 3, but new Channel careers can start only from rank 2.

For that reason, it's good to keep your Channel and Western Front careers on separate profiles, so you unlock new ranks only once per profile. 

 

"Unlock effect" is provided by existing careers as long as they exist. If you have only one career that has reached high rank and delete it, you'll lose ability to start new careers from that rank until you reach it hard way in other career. Same with decorations in main menu. 

 

Finally, merely getting promoted to new rank is not enough get unlock. You must get to the next day. That's because on day on your promotion you start with your old rank, and until the day ends, the game starts you at old rank then shows you promotion message when you enter the career. Once you get to the next day (and begin day with new rank), this career starts providing your new rank to all other careers. So if you read promotion message, exited the career immediately, tried to make a new one and it didn't have the rank available, that's why.
