The altimeter in the hud is relative to the actual ground height, I think... Try it over sea then over some hills/mountains to check.

Ehret is correct; the altimeter in the hud shows your height above the ground.

 

The altimeter in your picture shows either the height above your departure airfield (assuming you set it that way before you took off) or it shows your height above mean sea level under ICAO Standard Atmosphere (ISA) conditions.

 

You set your altimeter using the keybind for "Altimeter: reference pressure toggle". Technochat shows one of the following when you set your altimeter:

- "Altimeter reference pressure: nearest friendly airfield elevation" (your altimeter goes to 0, showing that you are on the ground)

- "Altimeter reference pressure: standard atmosphere" (your altimeter goes to your present height above mean sea level)

 

Note that the bombsight altimeter always shows "Altimeter reference pressure: standard atmosphere" (assuming that feature has not changed since the latest release). So, you need to set your bombing height from one of the following:

- The hud altitude (if it's available and reasonably close to your altitude once you drop)

- The cockpit altimeter set to "Altimeter reference pressure: nearest friendly airfield elevation" (if it's reasonably close to your altitude once you drop).

- A calculation based on knowing the actual height of the target above sea level.

  For example: Abs = Acurr - (Htgt - Hdep)

  where Abs is the bombsight altitude setting, Acurr is your current altitude above departure airfield (cockpit altimeter), Htgt is the height above sea level of the
  target, and Hdep is the height above sea level of the departure airfield.
