After a flight, return to menu with no options.

Playing in VR, so far I mostly tried out the quick mission option and it happens regularly, that once the game ends (usually with me dying), it loads back out into the hanger but the only screen in front of me is the darkened background rectangle and nothing else.  No buttons to press and nothing I can do to get to the options again and select another flight.

 

This has happened about 5x, pretty much each time I have played the game it happens at some stage, whether after the 1st flight, 2nd or 3rd. 

 

Today I thought let me try a campaign.  Selected one and loaded, the flight went as expected with me dying after 10mins and going back into the game I wanted to fly the same mission but there was only a map and no markers/airfields to choose from.  Just a blank map. 

 

I'm not sure if any of this is related or separate issues.  just wondering if anyone has experienced this and is there a fix?

I have seen it happen one time for me, not sure what the cause was.

Pretty sure it happened to me after I had died in a quick mission.
