Inexplicable spawn death

This is the second time it happens to me and I don't understand.

I just spawn and I'm dead before I even get in the plane apparently but the thing is my game froze completely and the only way to get out was to press ctrl+alt+delete...then I check the log and I see this...

Could this be some glitch?

One more thing I was stuck on the map never even seen the plane i tried to use.

And I just got another one exact same issue exact same plane but different map.
