...very weird behaviour... In MP or SP it doesn't matter whether balanced, high or ultra settings I use ingame - always almost the same fps: on the ground up to 30 fps, midair in low altitudes 30-40 fps and in in high altitudes 30-50 fps (depends from the number of other planes, ground sceneries etc.); CPU usage 50-80 % in average. And I can see no great differences in relation to the settings between balanced, high etc. (I didn't try low yet...), for me it looks always nice (ultra seems a little bit nicer). In doing so BOS is running very smoothly.
